How We Can Help
We offer a comprehensive range of services designed to promote independence, dignity, and quality of life for individuals with disabilities.
General Services

CDS Attendant Program
The Consumer Directed Services (CDS) Program allows you to hire your own personal attendant. You control who provides your care and how it's delivered. This program empowers individuals with disabilities to maintain independence while receiving the in-home support they need.

Agency Directed Services
Agency Directed Services provides professional in-home assistance managed by our trained and experienced staff. Services include help with daily living activities such as housekeeping, meal preparation, laundry and essential shopping.

Durable Medical Equipment
We maintain an extensive durable medical equipment program, including walkers, bath benches, and other adaptive items. Equipment is offered at affordable, reduced rates—often lower than traditional providers—with flexible pricing based on individual need.

Home Modifications
We help make homes safer and more accessible. From ramps and grab bars to other accessibility improvements, we work with individuals to identify needs and find solutions that support independence at home.

Community Transition
Helping individuals transition from nursing facilities back to independent community living with comprehensive planning and support.

Youth Services
